Create a Service Request

1. From the Requester console, click "Create a New Request".  (click on image to see a larger one)

The New Request wizard appears: (click on image to see a larger one)

2. Complete the required fields:

   a) Put in a one sentense summary of your request in the "Summary" field. ("Summary" field has a limit of 128 characters)

   b) Put in more details of the request in the "Notes" field, such as the name of the system, type of the operating system, and details of the problem etc.

   c) Select an "Urgency" level for your request from the list.

3. (Optional) Click "Add Attachment" to include an attachment, such as a screen shot, a log file, or an error report etc.

4. (Optional) Click on the calendar icon to select a date for "Date Required" — a date by when you require this request be completed.

5. Click "Save".
